As part of the process when booking your scan we request and require the following information:

Name and Date of Birth
Contact information including email address
Demographic information, such as address and postcode
Information relating to your clinical history for which you have already seen a health care professional for such as GP, Consultant, Physiotherapist.
Information regarding your any previous operations or implants in your body you may have had put in either surgically or through any accidental injuries. This information is only collected for the purpose of your safety for your scan

WHAT WILL MY PERSONAL DATA BE USED FOR?

Any information we collect from you will only be used for the purpose of your scan. This means personal data such as name, date of birth, address will only be used to ensure we can identify your scan , and radiology report to yourself only

Information regarding previous medical surgery or implants in your body is solely collected for your safety and to ensure you are safe to have your scan.

Images from your scan will be shared with our Radiologist for the purpose of producing a report for your scan. If any abnormality is found on your scan , your GP, consultant or any other specialist centre may request your scan images. We will only share this with your consent if requested.
